WIN/LOSS STATEMENTS

Player Extras Members are eligible to receive a win/loss statement by mail. Please click on the link at the bottom of this section and print out the attached document. Complete the form and mail to:

The information you provide on the form must match that of your account in order for a statement to be mailed to you.

The Win/Loss Statement will reflect our records of your card-in play for the year you request as of the date issued. The Statement is only an estimate, and is dependent on proper use of the Player Extras Card.  It includes slot play and horse wagering conducted with the Player Extras Card.
