Guests that are Player Extras members are eligible to receive a win/loss statement by mail. Please click on the link at the bottom of this section and print out the attached document. Complete the form and mail, along with a copy of your identification, to:

The Win/Loss Statement will reflect our records of your card-in play for the year you request as of the date indicated on the form. The Statement is only an estimate, and is dependent on proper use of the Player Extras Card.
